    For families who are interested in enrolling your child(ren) in Northport Homelink for the
    2023-2024 school year —WELCOME!

    1.   Program Registration form - Northport Homelink’s “Choice” Enrollment form
    2.   “Acknowledgement of Receipt and Understanding of Handbook”
    3.   Transfer of enrollment through the Washington State Electronic Choice Transfer
    4.    Respond to “Family Survey” when it is available.

     NOTE: Bring current other forms as individually applicable
    Enrollment documents to be submitted for new students:

    1.  Program Registration form - Northport Homelink’s “ChoiceEnrollment form

    2.  “Acknowledgement of Receipt and Understanding of Handbook”

    3.  “Internet Usage Agreement” (to be signed, handed in to the Northport Homelink office, and then it will be kept in each student's file)

    ○ Choose the age appropriate form…

    4.  Nurse Permission form - allows our school nurse to update immunization records (if needed) or is necessary to accept/enter a student's immunization exemption(s).

    5.  Evidence of student age: send a copy of a documentation confirming student age
    ○ (A document or other evidence that helps confirm the student's age, such as a birth
    certificate, hospital record, or physician's certificate showing date of birth; religious record
    of birth or an entry in a family Bible; an adoption record; an affidavit from a parent; previous
    school records; etc.)

    6.  Transfer of enrollment through the Washington State Electronic Choice Transfer
    site.  NOTE: Make sure to choose the School Year 2023-2024
    ○ Parents should go directly to the Parent Portal for Choice Transfer and initiate that transfer on the electronic transfer system - please remember that electronic requests for a Choice Transfer must be completed each school year (they automatically expire each school year).
    ○ Go to: eds.ospi.k12.wa.us/choicetransferrequest
    ○ School Year: “2023-2024 School Year”
    ○ Start Date: “9/1/2023”
    ○ End Date: “6/15/2024” or “8/30/2024”
    ○ Resident School District: (the district in which you reside)
    ○ Nonresident School District: “Northport School District”
    ○ Current or last School district attended: (select applicable district information)

    7.   Immunization records (IT IS ESSENTIAL THAT WE HAVE YOUR STUDENT’S
    CURRENT IMMUNIZATION RECORDS ON FILE – (please contact the Northport Homelink
    office if you have any questions or would like clarification of alternatives).

    Keep your child’s vaccination records in a safe place beginning with their first vaccine. Your child’s doctor will keep a record of vaccines given in their clinic. Ask your child’s doctor or nurse how they keep vaccination records and how you can get an official copy. You can also ask your child’s doctor to enter vaccination information in your state’s immunization information system (IIS), a statewide registry for records.
    If you can’t find a copy or do not have your child’s vaccination records, you can try to get an official copy from a few different places.
    Your child’s doctor or clinic
    Doctors and health clinics keep records of the vaccines they give to your child. Keep in mind that they may save records for only a few years. If your child has gotten vaccines at different locations, you may have to contact each of them to get your child’s records.
    Your state’s immunization registry
    States and some large cities have secure immunization information systems (IIS) that keep vaccination records for the people in the state or city. However, some people’s records may not be complete. Contact the IIS in your state or the state where your child received their last vaccine to see if they have your child’s records and learn how to request an official copy. Your doctor may also be able to help you get vaccination records from your state’s IIS for vaccines that were given at other locations by other providers.
    NOTE: IMMUNIZATION EXEMPTION information may be requested from the Northport Homelink office
    8.   “2023–2024 Family Income Survey” - This survey is used by the Northport School
    District to help maximize available funding from state and federal sources, as well as to
    provide certain other benefits that may be available for your children. This survey will
    come available during the summer or early fall and will then be sent to enrolled
