RETURNING STUDENTS ONLY FILL OUT AND SEND IN:
1. Northport Homelink program-specific basic enrollment paperwork for the 2024-2025 school year
2. “Acknowledgement of Receipt and Understanding of Handbook”
3. Transfer of enrollment through the Washington State Electronic Choice Transfer (for students who reside outside of the Northport School District’s geographical boundaries).
4. Respond to “Family Survey” when it is available.

NEW FAMILIES RESIDING OUTSIDE OF NORTHPORT SCHOOL DISTRICT:
1. Simply go to the Washington State Electronic Choice Transfer site.
Parents should go directly to the Parent Portal for Choice Transfer and initiate that transfer on the electronic transfer system - please remember that this electronic request for a Choice Transfer must be completed each school year (they automatically expire each school year).

- Start Date: 9/1/2024
- End Date: “6/15/2025” or “8/30/2025”
- Resident School District: (the district in which you reside)
- Nonresident School District: “Northport School District”
- Current or last School district attended: (select applicable district information)
- Reason for Request: “To enroll in an alternative school/program”
2. Parents of these new students will be contacted by one of our Homelink teachers (usually within 7-10 business days) to answer any questions, to clarify expectations, and to have a follow-up conversation.
NEW FAMILIES RESIDING WITHIN NORTHPORT SCHOOL DISTRICT:
1. The first step for next year's Northport Homelink enrollment is for the parent/guardian of an inquiring new local student to contact the Northport Homelink office to request or initiate a start of the enrollment process.
Parent/guardian will provide:
- contact information of their own name, phone number, email;
- student information of name, grade, date of birth, address
- reason for request
- any other information that the parent/guardian believes may be helpful in placement.

Requests for enrollment will be forwarded to our principal, Dr. Hunt. She reviews new enrollment each Wednesday and assigns a teacher contact or a “Success Team Meeting”.
The second step for local families who are new to the Northport School District is that they will be contacted by one of our Homelink teachers (usually within 7-10 business days) to answer any questions, to clarify expectations, and to have a follow-up conversation.
The second step for students requesting a transfer from our local brick and mortar school to Northport Homelink enrollment is that a Success Team Meeting will be scheduled to discuss transfer.
This step is required prior to the transfer being approved.
THE THIRD STEP FOR ALL NEW FAMILIES will be to fill out the program-specific Northport Homelink new-enrollment paperwork and send it in to the Homelink office.
This paperwork and instructions can be found on the Northport School District website …or can be sent from the Homelink office
- Program Registration form - Northport Homelink’s basic Enrollment form
- Acknowledgement of Receipt of Handbook
- “Internet Usage Agreement” (to be signed, sent in to the Northport Homelink office, and then it will be kept in each student's file)
- “Nurse Permission form” - allows our school nurse to update immunization records (if needed ) or accept and enter immunization exemption(s).
- Evidence of student age: send a copy of a documentation confirming student age (A document or other evidence that helps confirm the student's age, such as a birth certificate, hospital record, or physician's certificate showing date of birth; religious record of birth or an entry in a family Bible; an adoption record; an affidavit from a parent; previous school records; etc.)
- Immunization records - IT IS ESSENTIAL THAT WE HAVE YOUR STUDENT’S CURRENT IMMUNIZATION RECORDS ON FILE. (Please contact the Northport Homelink office if you would like information on the option of Immunization Exemptions.)

KEEPING TRACK OF RECORDS
Keep your child's vaccination records in a safe place beginning with their first vaccine. Your child's doctor will keep a record of vaccines given in their clinic. Ask your child's doctor or nurse how they keep vaccination records and how you can get an official copy. You can also ask your child's doctor to enter vaccination information in your state's immunization information system (IIS), a statewide registry for records.
FINDING OFFICIAL VACCINATION RECORDS
If you can't find a copy or do not have your child's vaccination records, you can try to get an official copy from a few different places.
Your child's doctor or clinic
Doctors and health clinics keep records of the vaccines they give to your child. Keep in mind that they may save records for only a few years. If your child has gotten vaccines at different locations, you may have to contact each of them to get your child's records.
Your state's immunization registry
States and some large cities have secure immunization information systems (IIS) that keep vaccination records for the people in the state or city. However, some people's records may not be complete. Contact the IIS in your state or the state where your child received their last vaccine to see if they have your child's records and learn how to request an official copy. Your doctor may also be able to help you get vaccination records from your state's IIS for vaccines that were given at other locations by other providers.
NOTE: IMMUNIZATION EXEMPTION information may be requested from the Northport Homelink office
NOTE: Enrollment will remain open through the first half of the school year (dependent on student seat availability and other outlined criteria).
However, after February 1, 2025, applications will no longer be accepted for the 2024-2025 school year - but there will continue to be the option of submitting applications for the following school year.
